Rec Sports Facility Waiver

Parents/guardians completing waivers for students under the age of 18 can email the signed waiver to Bob Petrowski, Director of Operations, at robert.petrowski@mines.edu. You will receive a confirmation once the waiver has been received and the student's account has been updated.
